Parent Permission Forms

At Calvary youth we take the safety and security of your youth very seriously. Please ensure that you have filled out these forms before allowing your youth to participate in our programs. These forms cover your student for September 2021 - September 2022. The safety of your youth is of the utmost importance to us and having the proper information allows us to give the highest level of care possible.

Parent Consent Long Form

If this is the first time you are registering your youth for one of our programs please use the Long Form.

Parent Consent Short Form

If you have registered your youth in previous years using the Long Form please use the Short Form to register.
